Covid 19 Vaccinations for Age 12 to 15yrs old - 4th November 2021

Dear Parents and Carers

The Immunisation Service would like to invite your child for a Covid 19 vaccination on Thursday 4th November, here at Sewell Park Academy. These will be carried out by the NHS Immunisation Team during the school day.

In order for your child to participate you will need to provide consent, via an online form, no later than midnight on Wednesday 3rd November.

They have also requested that you complete the form if you wish to decline the offer, by selecting do not consent on the consent section of the form.

Please note the HPV vaccinations previously scheduled for this day have been postponed and you will receive further information when these have been rearranged.

If you have any further questions about vaccinations, or are unable to complete the online consent form please contact the Imms Team on 0300 555 5055. Please do not contact the school as we will not be able to answer queries relating to vaccinations.
